• CareFacts’ HIS software platform offers over 200 healthcare agencies a networked PC-based, clinically driven application suite designed to address all aspects of patient interaction including scheduling, patient charting, reporting and billing.
• The CareFacts’ software incorporates a standard nursing and healthcare language with over 150 outcome assessment tools, which may be customized based on the needs of nurses, therapists and other clinicians.

Corporate History

CareFacts Information Systems, a privately held corporation headquartered in St. Paul, Minnesota, provides PC-based clinical and administrative healthcare information systems for the home healthcare, public health and hospice care segments. CareFacts was incorporated in Minnesota in 1992. Since its inception, the Company has established a customer base of 200 clients in 25 states and expanded into an organization of 39 employees.

To date, CareFacts has devoted substantially all of its efforts to the research and the development of its healthcare information system. Since inception, CareFacts has focused on providing superior systems to facilitate community-based remote care, eventually releasing CareFactsTM Clinical in 1995. After the release of the original Clinical software application, it became apparent that more modules were needed to provide an integrated community-based care software platform. In August 1998, CareFacts merged with Sands Business Solutions, a regional provider of billing and scheduling systems to community-based care agencies, creating the desired integrated HIS software platform for the community care market.

Shortly after the merger, CareFacts rewrote the software so that it incorporated Clinical, Scheduling, and Billing in a Windows platform using a single integrated database. In 2002, CareFacts added the Home Health Aide PDA Charting Module.

CareFacts was named the 5th-fastest-growing technology company in the state of Minnesota and the 163rd-fastest-growing technology company in North America by Deloitte & Touche, Forbes ASAP and the Minnesota Technology Association. We have continued to be recognized for being among the most rapidly growing technology companies in Minnesota for the last five years, while retaining profitability.
