CareFacts™ Clinical

CareFacts™ Clinical is designed with clinicians in mind. The purpose of software is to enable clinicians to easily enter information so their main focus in on the patient, not the computer.

Clinical operates as the entry point for information in the CareFacts HIS, integrating with Scheduling and Billing functionality through the CareFacts™ Courier repository. Laptops equipped with Clinical (client-based) allow nurses and other clinicians to remotely document each care encounter directly at the point of care; this data is then uploaded to the Courier (server) database. The Clinical foundation is built on the Omaha System and OASIS Data Set – recognized standards in nursing documentation language. Clinical’s charting functionality is designed according to traditional care provider workflow and documentation and may be customized as desired. Combining the Clinical standard language with a menu driven charting tool enables nurses and other clinicians to consistently and easily document both simple and highly complex patient cases.

Clinical provides nurses and other clinicians a modular approach to generating each patient care plan by leveraging stored data sets with predefined, customizable patient assessment pathways. Care plans are updated based on outcome assessment reports generated by Clinical. Essentially, the system functions as a disease management tool, helping community-based care agencies improve patient outcomes while reducing the cost of care.
