CareFacts™ Hospice

Our hospice features, embedded within our already proven system, include LMRP guidelines, hospice certifications, level of care tracking, multi-disciplinary care planning/ pathways, interdisciplinary team meeting documentation, and specialized hospice assessments such as pain, wound, spirituality and nutrition. Hospice bereavement can be scheduled and documented as completed.

Our Clinical software works for either hospice only agencies or in combined homecare/ hospice agencies. Each client admission is unique to home care or hospice if an agency has both businesses. Hospice business rules are enforced for certification periods with warnings for the clinician about due dates. Our Clinical software includes automation of agency – specific Standing Orders/ Protocols. Because the entire hospice team can view the charts, there is improved coordination.

CareFacts™ offers a variety of management reports for hospice. This includes schedules; time & activity tracking by staff, discipline, and program and/ or geographical area; and, financial reports. Other features include, tracking of time by volunteers, length of stay, level of care, referral data for marketing reports, and unduplicated census statistics.

Along with our Clinical System, CareFacts™ offers many advantages for hospice with our Billing and Scheduling Systems. With our Scheduling Software, you can graphically schedule paid staff and volunteers and create care plan/ documentation forms for paraprofessionals and volunteers. Bereavement follow-up activities such as mailings, follow-up visits/ calls and newsletters are scheduled and confirmed. Volunteer hours are tracked in the system. With our Billing software, you can bill any pay source for hospice. Per diem expenses are automatically generated based on patient’s level of care. CareFacts™ Billing software allows you to electronically file for hospice payors.
