CareFacts Products & Services

The CareFacts HIS software platform is a PC-based clinical and administrative healthcare platform for the home health, public health, and hospice care segments – collectively, the community-based care market. The platform offers healthcare agencies a clinically driven application suite designed to address all aspects of patient interaction including scheduling, patient charting, reporting and billing. The client-server application enables nurses, other clinicians (using laptops or notebooks), and para-professionals (with inexpensive PDAs) to document each patient encounter and update information at the point-of-care in multiple care settings. CareFacts’ software incorporates standard nursing and healthcare language with over 150 outcome assessment tools. These tools are critical to the development and modification of a care plan based on the patient’s health status and may be customized based on the needs of its nurses and other clinicians in order to improve patient outcomes and eliminate unnecessary costs in providing care. The HIS system consists of four client side modules: CareFacts™ Clinical, CareFacts™ Scheduling and CareFacts™ Billing plus a streamlined PDA module, CareFacts™ PDA, which is optimized for paraprofessionals. Additionally, CareFacts™ Courier, a server side repository, ensures data integrity and consistency throughout the system. These modules work as a cohesive, fully integrated unit that enables community-based healthcare agencies to deliver comprehensive care more efficiently and effectively. These tools, in turn, allow healthcare institutions to:

  • reduce the time and cost of managing care to each patient
  • increase total patient capacity
  • enhance nurse or other clinician productivity
  • minimize administrative overhead by automating scheduling, billing and reporting functions
  • reduce scheduling, documentation and billing errors
