CareFacts™ Public Health

The CareFacts™ system includes a version designed specifically for Public Health. Modules available include CareFacts™ Clinical Charting, CareFacts™ Scheduling and CareFacts™ Billing, which are integrated into a single database.

The backbone for assessing, care planning, documenting, and demonstrating outcomes is based on the Omaha System. The Omaha System is one of the ANA nationally recognized standardized languages. Care plans can be created for each client or pathways can be selected to provide consistent care.

CareFacts™ includes approximately 140+ more detailed assessments as specific supplements to enhance the community-based appear of the Omaha System. The program is used by public health agencies in such programs as home visits (maternal-child, medication compliance, adult health promotion, etc.), communicable disease follow-up, clinics, and comprehensive time/ activity tracking. The CareFacts™ system has the flexibility to develop additional specific assessments for grants or special programs.

Since CareFacts is a client-server program, multiple staff and/or disciplines (RN’s, LPN’s Social Workers, etc.) can have the chart simultaneously and all users read or chart as needed.

Our system has multiple security levels within the system to accommodate the “minimum necessary information needed” to protect client confidentiality. CareFacts™ meets the HIPAA requirements.

CareFacts Scheduling is useful for the flexible planning of schedules for follow-up visits with clients. The Management Report Writer, included in CareFacts, allows agencies to run reports required for public health statistics, as well as outcome reports for programs and groups of clients. This can be particularly useful for tracking grant requirements. All management reports can be viewed, printed, or automatically exported to Excel.
